1. The application for operation license in form No. 01 in Annex XI enclosed with Decree 109/2016/NĐ-CP of Vietnam Goverment;

2. A valid copy of an investment certificate applicable to foreign capital specialized clinic;

3. Valid copies of practice certificates of chief physicians of the specialized clinic; persons taking charge of professional department of the specialized clinic;

4. The list of registration practitioners at specialized clinic (including registration of practitioners and specialized medical workers at the clinic but not subject to the granting of practicing certificates) in form in Annex IV enclosed with Decree No. 109/2016/NĐ-CP of Vietnam Goverment;

5. List of material facilities, medical equipment, organization and personnel of specialized clinic in form No. 02 in Annex XI enclosed with Decree No. 109/2016/NĐ-CP of Vietnam Goverment;

6. Documents proving that the specialized clinic satisfies the requirements for facilities, medical equipment and organization suitable for the practice scope of any healthcare facility form specified in section 1 Chapter III of  Decree No. 109/2016/NĐ-CP of Vietnam Goverment  Click (see more details);

7. A professional and technical list proposed by specialized clinic on the basis of the professional and technical list issued by the Minister of Health;
